The Spring 2017 PM Academy Workshop programme is online. The programme is entitled ‘Working Better Together: How to build multidisciplinary and multiagency teams to improve patient care’ and consists of whole day courses taking place at 10 locations around the United Kingdom. This unique PM Academy programme will focus on how to deliver projects requiring the input of multiple teams or organisations.
The Academy Workshop in Northern Ireland will take place on Tuesday 23 May 2017 at the Maldron Hotel Belfast, Belfast International Airport, Belfast BT29 4ZY from 10am to 4pm.

It was the first ever conference focused solely on Respiratory Medicines Optimisation. The National Medicines Optimisation in Respiratory Medicine Workshop was a joint meeting organised by Pharmacy Management in partnership with UKCPA.
The Journal of Medicines Optimisation (JoMO) has worked with the Respiratory Specialist Interest Group of the UK Clinical Pharmacy Association (UKCPA) to create a day of satellite sessions that will deliver quality learning quality and best practice exchange for all sectors of pharmacy. MOIC presented its mission, projects and programmes at the conference.

During a 2 day meeting of the MOIC/Digitalis team with their counterpart at the Gdanski Uniwersytet Medyczny (Gdansk Medical University) the participants worked on introducing STEPSelect technology in the hospital. STEPSelect is a combination of a knowledge system (MATRIX productions) and an instrument to induce consensus decision making about therapeutic choices (webbased SOJA methodology). The teams developed more than 40 different MATRIXes on different therapeutic classes, to be used at the UCK (Uniwersyteckie Centrum Kliniczne, the University Clinical Centre in Gdansk).
Pilot LMWHs
The next step is a pilot with STEPSelect on LMWHs (Low Molecular Weight Heparin, a class of anticoagulant medications) using the MATRIX as developed in Northern Ireland. The pilot will be a joined effort of UCK (Tomas Stefaniak, Pawel Faczynski), MOIC (Frans van Andel, Michael Scott) and Digitalis (Rob Brenninkmeijer, Rob Janknegt). In February at least 20 clinicians at UCK will be trained in LMWH MATRIX. Target is to finalise, execute, evaluate and introduce the LMWH project by October 2017.

Mike Scott, Director MOIC, and Mark Lee, with the European Reference Site Award. The ceremony took place during the second European Summit on Digital Innovation for Active and Healthy Ageing, Brussels, 6-7 December 2016. Northern Ireland achieved the highest recognition of 4 Star Reference Site Status from the European Innovation Partnership on Active and Healthy Ageing (EIP on AHA).

November 2016 was successful for Antrim Area Hospital and MOIC. Professor Michael Scott (Director MOIC) received the UKCPA Lifetime Achievement Award 2016 at the UKCPA Conference at the Hilton Hotel Manchester Airport, 4 November 2016. Julie Magee, pharmacist at Antrim Area Hospital, was the winner of the best poster at the Hospital Pharmacy Europe Live, 10 November 2016, at the London Olympia Conference Centre. Seamus O’Reilly, Medical Director NHSCT, congratulated Professor Michael Scott and Julie Magee.
Doctor Light Discharge
Julie Magee has won the best poster for ‘Doctor Light Discharge’. Entries were open to hospital pharmacists from across the UK and Europe working on innovative research and pilot projects. Julie’s winning poster will be featured the Hospital Pharmacy Europe Journal in January 2017.
After a patient is deemed medically fit there can be delays before a doctor can prepare the discharge summary. This can be due to medical staff attending ward rounds or carrying out other duties. The aim of this study was to reduce delay in the discharge process by introducing a ‘Doctor Light Discharge’. This involved the Pharmacist Independent Prescriber preparing the discharge summary and medication section. Data was collected on Ward A1 before and after ‘Doctor Light Discharge’ summaries were introduced. Time was recorded from the time pharmacy was informed of discharge until the discharge summary was written. The time reduced from a mean of 127 minutes when the medical staff was preparing the discharge summary to 33 minutes when it was prepared by the pharmacist. Statistical analysis, carried out using Mann-Whitney U test, indicated that the discharge summaries were ready significantly more quickly during the intervention.
An audit was carried out on the ‘Doctor Light Discharge’ summaries and showed that in 100% (25/25) of the letters prepared by the pharmacist all information required was present and correct. The results of this study show that a pharmacist independent prescriber can generate an accurate, complete and timely discharge summary.

Of course MOIC was present at the annual flagship event of Pharmacy Management.The theme was ‘Medicines Optimisation: in long term conditions, the Frail and the Elderly’. Many of these patients suffer from multiple long term conditions which makes their treatment much more complex. With increasing age the potential benefit reduces but the risks from treatment increase, suggesting that regular review and appropriate de-prescribing is likely to be in the patient’s interest.
