iSIMPATHY (Implementing Stimulating Innovation in the Management of Polypharmacy and Adherence Through the Years) was a 3.5year €3.5m project funded by the European Union’s INTERREG VA Programme and managed by the Special European Union Programmes Body (SEUPB).
The project was developed to enhance medicines use across Northern Ireland, Scotland, and Ireland. It trained pharmacists and healthcare professionals to deliver 6,481 person-centred medicines reviews (1,905 in NI) using the 7-Steps methodology, embedding shared decision-making for polypharmacy management. An accredited training programme enhanced skills, while the iSIMPATHY model, recognised globally, improves patient outcomes, safety, and engagement through a “what matters to me” approach, aligning with Scottish polypharmacy guidance. It has sparked EU and global interest for addressing this public health challenge.
