Big Discussions : Pharmacy’s Role in Strengthening Winter 2025 Care Pathways

The Chief Medical Officer and Chief Nursing Officer have led four impactful “Big Discussion” workshops, engaging stakeholders across Health and Social Care to reflect on Winter 2024 and plan actionable steps for Winter 2025. Key focus areas include enhancing care pathways for older people, frailty, and community-based alternatives to hospital care.

Yesterdays “Big Discussion” workshop, hosted by MOIC and the Department for Health, zeroed in on pharmacy’s vital role in the whole-system flow plan for Winter 2025. Our goals?

• Define how pharmacy teams across all sectors can prevent and manage medication-related issues for older people, frailty, and care home residents—addressing falls, fractures, polypharmacy, and end-of-life care.
• Identify immediate improvements for Winter 2025/26 within existing resources, plus longer-term service development opportunities.

Exciting ideas were generated, and key improvement areas identified! Stay tuned for the report detailing how pharmacy will drive better outcomes this winter.
